SERVICE PROVIDER REQUIREMENTS

To get started with SDPS Pay, please follow the steps below, and upload required documents. The sooner we have everything needed, the sooner you can begin providing services!

ALL Providers Must Complete the Forms Below:

Step 1: Read the Provider Welcome Letter

Step 2: Complete the Service Provider Info Card

Step 3: Sign the EVV Acknowledgement Form

STEP 4: Choose Your Payment Method:

Sign up for EBT or CHECK Payments
